
​​Form U Instructions
Please follow the instructions herein. For questions regarding what can be disposed of at our sites, please navigate our website or call (570) 297-4177. When finished, please email asheets@ntswa.org & we will submit your information to DEP for disposal approval.

Step 1) Read the following general information.
Payment - We can accept cash or card (card itself must be present on delivery) at the scalehouse. We offer account creation to businesses that frequently/recurringly use our services. If interested, please fill out and return the Credit Application. Please choose references that will be quick to respond & you know will vouch for you.
Response Time - Though generally responsive within a week, DEP may take up to 3 weeks to process your submission. We will notify you as soon as they approve or reject the submission.
Analytical - Some wastes require analysis by a PA DEP accredited laboratory prior to disposal. See the R-Tables for an explanation of your sampling & testing requirements.
High Density Waste - Be advised that high density and/or saturated & absorbent materials (soils, drill cuttings, sands, etc. if hauled in a roll-off container, may only occupy up to ~50% of the container's volume to avoid an overweight load)
Act 90 - If you haul waste to any PA waste processing or disposal facilities using a truck registered with a Gross Vehicle Weight greater than 17,000 lbs or a trailer registered with a GVW >10000 lbs, PA DEP requires the hauler has an Act 90 sticker (proof of a Waste Transporter Authorization) for that truck and/or trailer. Information on obtaining one is on PA DEP's website. The law prohibits processing and disposal facilities from accepting waste from waste transportation vehicles that do not have a valid authorization, including vehicles with expired authorization stickers and cab cards. If your registration(s) are under the limits, you may disregard; If you are over the limit but do not want to obtain an Act 90 sticker, you may transport the load in a lighter vehicle or trailer.

Step 2) Complete general forms.
All Applicants:
Complete the generator & hauler sections of the STWPA. This is our internal billing & records form.
Unless disposing of virgin fuel contaminated soil, sign & date the Non-Hazardous Waste Certification Form, which verifies that you do not & will not send hazardous compounds to the landfill.
If you are currently a PADEP client & have an ID, you do not have to complete a Client Add Form. If unsure, please complete the top section of the form.
Unless you're disposing of waste tires or asbestos, complete the Waste Manifest & give copies to your hauler(s) to present at the landfill scales.
Unless disposing of sewage sludge, please create a detailed description and/or schematic of the waste generation process in a separate document.
Step 3) Complete the forms below that coincide with what you wish to dispose of.

Plant Trash
Complete the Form U - Plant Trash.
Plant Trash is various waste generated from most facilities. Form U's are required if you generate over 1 ton of waste annually. If you generate any of the items listed on page 2 section 4 of the Form U, you will be waived from having to submit any lab reports for chemical analysis. Please complete Section C, Sec. D, Sec. D1c, & Sec. D4.
Virgin Fuel Contaminated Soil
Fill out Sections B, C, & D & return Form FC-1. If contaminant type is an 'other' (e.g. hydraulic fluid) Please include MSDS for the spilled contaminant.
Lab testing parameters can be found in the FC-1 instructions. If the volume of contaminated soil is <25 cubic yards, no testing is required. If the volume is <30 cubic yards, an acceptable cost-saving deviation from said instructions would be to only sample for total petroleum hydrocarbons (TPH).
Contaminated soils are to be tested per clean-up event, per generator location. FC-1's are for spills of virgin petroleum-based fluids only.
Other Contaminated Soil
Complete the Form U-CS. Sampling parameters are in Attachment I of the R-Tables. Testing parameters are in the highlighted portion of Page 1.

Friable Asbestos Containing Waste
Complete the Asbestos Manifest & give copies to your hauler(s) to present at the landfill scales.
Complete the Form U - Asbestos.
Tires
Please complete Section C of the Form U - Tires & the top section of section D - the amount of tires & the frequency. We estimate a car tire is 20 lbs. So, if you dispose of 100 car tires per year, we ask you enter 1 ton annually in section D.
To properly dispose of waste tires, you must have DEP & Landfill approval to cut the tires to put them in the trash. Waste tires are to be cut in half (2 pieces) or both sidewalls removed from the tread portion of the tire. Once the tires are properly cut & approval is granted the tires are considered municipal waste & can go in your regular trash.
If hauling, please complete the Waste Tire Manifest.

Horizontal Directional Drilling Waste
If you wish to dispose of both wet (does not pass the paint filter test), & dry HDD waste, please complete a Form U - HDD Waste, Non-Haz Certification, STWPA, & separate manifests as needed, for both types of waste.
An analytical may be waived by DEP if at each drill site, for every property overtop the source of waste generated, be it 1 or more, the property owner/resident/knowledgeable official completes the Non-Contaminated Near Surface Certification. Alternatively, use sampling & testing parameters for Other Contaminated Soils found above on this page.
Sewage Sludge
Form 43's and the required lab testing are valid for up to 5 years from one site. If your plant undergoes a change in treatment process or major upset in treatment quality within the 5 years, DEP requires new testing & submission of another Form 43 prior to disposal.
Please read the Form 43 instructions. SECTION D2, a & b describe the testing parameters for disposal at our facility. Testing must be done by an accredited laboratory.
Please complete Sections C-H of the Form 43.
